Empowered Midwifery Care PLLC is currently considered in-network with

If your insurance is not listed above, we may be considered an out-of-network provider. We will be more than happy to see you but you will be responsible for charges at the date of service. We are happy to submit that claim to your insurance company for you. We offer reasonable out-of-pocket fees for annual gynecological care and services. Additionally, we provide student discounts, medicaid eligible discounts, and hardship discounts.
Explanation of Billing
Payment: Payment is due at the time services are rendered. We accept cash, credit cards, and checks. Our office will bill your insurance company for your visit. We will collect all co-pays, deductibles, or co-insurances at the time of your visit. It is ultimately your responsibility to know your individual insurance policy’s terms.
Obstetrical care
Empowered Midwifery Care PLLC does charge a midwifery fee which is a separate bill from what we bill your insurance. This fee covers the services provided by Empowered Midwifery Care PLLC that are not covered or billable by most insurance companies. We offer payment plans and discounts for medicaid eligible as well as hardship discounts. These discounts are arranged on an individual basis.
